Kyle McCormick, PharmD ’14, has been awarded a grant by the Pennsylvania Pharmacists Association for project entitled Patient Activation as a Measure of Medication List Accuracy. Faculty collaborators on the project include PittPharmacy Senior Associate Dean Randall Smith,PhD, Research Fellow Jennifer Bacci, PharmD, BCACP, student pharmacist, Alyssa Kunkely (class of 2016), and residency preceptor and owner of Gatti Pharmacy, Stephanie Smith Cooney, PharmD.
The purpose of this project is to understand whether their is a relationship between patient activation, as measured by the Patient Activation Measure (PAM) survey, and the number of discrepancies between provider medication lists. Understanding the relationship between patient activation and medication list accuracy may provide insight into causes of error in transitions of care. We hypothesize that as a patient becomes more engaged with their health, as evidenced by a higher PAM score, the medication lists among their practitioners will be more accurate as a result of increased communication. The PAM score may be a way to triage patients who need assistance from pharmacist-provided coaching. Additionally, the project aims to identify ideal means of communicating updated medication lists to providers via a provider survey.
