Sandra Kane-Gill, PharmD, MS, FCCM, FCCP is the guest editor for the Pharmacotherapy July 2018 themed issue on innovations in medication safety. Innovations in medication safety in all patient care areas including the hospital, nursing home and the community are addressed. The issue addresses a range of approaches including mining big data for post-marketing surveillance, the use of natural language processing, and incorporation of technology to prevent medication errors and adverse drug events. Managing alert and alarm fatigue and recommendations for managing alarm fatigue associated with smart pumps are also included. Drug-related hazardous conditions and use of predictive algorithms are the focus of several articles.
